Kentucky Health Center Network (KHCN) announced it has selected i2i Systems, a population health management and analytics provider to improve health outcomes across its Health Center Network. Implementation of i2i Systems’ PopIQ software will be used to aggregate de-identified patient data across multiple organizations, which allows for comparative analysis among KHCN and its clinic sites.

The information will help KHCN identify promising and best practices that could be replicated in other health centers. It also allows users to generate reports and dashboards across a virtually unlimited number of measures and indicators.

KHCN plans to fully deploy PopIQ in each of its member Health Center organizations, with information provided from its 161 patient care sites.
